2. What is ALERWITH-M used for?

ALERWITH-M combines an antihistamine with a leukotriene receptor antagonist.

It may be prescribed for selected allergic conditions such as:

  • Allergic rhinitis
  • Seasonal allergic rhinitis
  • Perennial allergic rhinitis
  • Sneezing
  • Runny nose
  • Nasal itching
  • Allergy-associated watery or itchy eyes
  • Selected patients with allergic airway disease, when montelukast is considered appropriate

Fexofenadine is an antihistamine used for symptoms associated with allergic rhinitis, while montelukast is used for allergic rhinitis and asthma in appropriate patients.

Important: The exact approved indication for the Fexofenadine + Montelukast fixed-dose combination should be confirmed against the current Indian product licence and prescribing information before making specific promotional claims.

3. How ALERWITH-M Works

Fexofenadine Hydrochloride 120 mg

Fexofenadine is a second-generation H1-antihistamine.

During an allergic response, histamine contributes to symptoms such as:

  • Sneezing
  • Nasal itching
  • Runny nose
  • Watery eyes
  • Itching

Fexofenadine blocks peripheral H1-histamine receptors and thereby helps reduce these histamine-mediated symptoms.

It is generally considered a relatively non-sedating antihistamine, although individual patients can experience dizziness or drowsiness.

Montelukast Sodium 10 mg

Montelukast is a leukotriene receptor antagonist.

Leukotrienes are inflammatory mediators involved in allergic and airway responses. Montelukast blocks the CysLT1 receptor, reducing leukotriene-mediated effects.

Montelukast can be used for allergic rhinitis and as part of long-term asthma management in appropriate patients. It does not act as a rescue medicine for an acute asthma attack.

4. Combined Mechanism of Action

ALERWITH-M addresses two important pathways involved in allergic symptoms:

Histamine pathway
→ Fexofenadine
→ H1-receptor blockade
→ Reduction of histamine-mediated symptoms

Leukotriene pathway
→ Montelukast
→ CysLT1-receptor blockade
→ Reduction of leukotriene-mediated inflammatory effects

This provides a complementary pharmacological approach for appropriately selected patients.

5. Therapeutic Benefits

When prescribed appropriately, ALERWITH-M may help provide:

  • Relief from sneezing
  • Reduction in runny nose
  • Reduction in nasal itching
  • Relief from allergy-associated symptoms
  • Reduction in histamine-mediated allergic manifestations
  • Reduction of leukotriene-mediated inflammatory activity
  • Symptom control in selected allergic rhinitis patients

The therapeutic response can vary from patient to patient.

6. Common Clinical Applications

Allergic Rhinitis

Allergic rhinitis can cause:

  • Sneezing
  • Runny nose
  • Nasal congestion
  • Nasal itching
  • Watery eyes
  • Throat or nasal irritation

Fexofenadine helps control histamine-mediated symptoms, while montelukast acts on the leukotriene pathway.

Seasonal Allergies

Seasonal allergens such as pollen may trigger recurrent nasal and ocular symptoms. Treatment should be selected according to the patient’s clinical presentation.

Perennial Allergic Rhinitis

Patients exposed to year-round allergens may experience persistent or recurrent symptoms. A physician may consider combination treatment in selected cases.

Allergic Airway Disease

Montelukast has a role in long-term asthma management for appropriate patients, but it should not be used as a substitute for rescue treatment during an acute asthma attack.

7. Suggested Use & Dosage

ALERWITH-M should be taken according to the physician’s prescription.

Each tablet contains:

  • Fexofenadine Hydrochloride IP — 120 mg
  • Montelukast Sodium IP — equivalent to Montelukast 10 mg

A 120 mg once-daily dose is an established dosage of fexofenadine for allergic rhinitis in commonly used product labeling. However, the dosing schedule for this particular fixed-dose combination should follow its approved Indian prescribing information rather than being inferred solely from the artwork.

Do not increase the dose or frequency without medical advice.

8. How to Take ALERWITH-M

  • Take the tablet exactly as prescribed.
  • Swallow with water.
  • Do not crush or chew unless specifically instructed.
  • Take with water rather than fruit juice.
  • Follow the timing recommended by your physician.
  • Do not take an additional tablet to compensate for a missed dose unless advised by your healthcare professional.

Fexofenadine exposure can be reduced when taken with certain fruit juices, including apple, orange and grapefruit juice.

9. Important Fexofenadine Interactions

Aluminium/Magnesium Antacids

Aluminium- or magnesium-containing antacids can reduce fexofenadine absorption. They should not be taken at the same time as fexofenadine; appropriate separation should be followed according to the prescribing information.

Fruit Juice

Apple, orange and grapefruit juices may decrease fexofenadine exposure. Taking the tablet with water is recommended.

Other Medicines

Inform the doctor about all medicines, supplements and herbal products being taken.

10. Possible Side Effects

Fexofenadine-related

Possible adverse effects may include:

  • Headache
  • Dizziness
  • Nausea
  • Fatigue
  • Mild gastrointestinal discomfort
  • Drowsiness in some individuals

Montelukast-related

Possible effects include:

  • Headache
  • Abdominal discomfort
  • Nausea
  • Diarrhea
  • Upper respiratory symptoms

However, montelukast has an important additional safety consideration concerning neuropsychiatric adverse effects.

11. Important Montelukast Safety Warning

Montelukast carries an FDA Boxed Warning concerning serious neuropsychiatric events.

Patients and caregivers should watch for new or worsening:

  • Agitation
  • Irritability
  • Aggressive behaviour
  • Anxiety
  • Depression
  • Abnormal or vivid dreams
  • Sleep disturbances
  • Confusion
  • Hallucinations
  • Restlessness
  • Memory problems
  • Suicidal thoughts or behaviour

FDA advises patients to stop montelukast and contact a healthcare professional immediately if new neuropsychiatric symptoms or suicidal thoughts/behaviour occur. Such events have been reported in patients with and without a previous psychiatric history.

For allergic rhinitis specifically, FDA recommends reserving montelukast for patients who have an inadequate response to or cannot tolerate alternative allergy medicines.

12. Precautions & Warnings

Before taking ALERWITH-M, inform your doctor if you have:

  • Kidney disease
  • Liver disease
  • Previous drug allergies
  • A history of psychiatric or behavioural symptoms
  • Asthma or other respiratory conditions
  • Pregnancy or plans to become pregnant
  • Breastfeeding
  • Any other significant medical condition

Kidney Disease

Fexofenadine is substantially eliminated through the kidneys, and dosage may need adjustment in patients with impaired renal function.

13. Pregnancy & Breastfeeding

If you are pregnant, planning pregnancy or breastfeeding, consult your healthcare professional before using ALERWITH-M.

The combination should be used when the treating physician considers the expected therapeutic benefit appropriate.

14. Use in Children

The artwork shows an adult-strength combination containing:

Fexofenadine 120 mg + Montelukast 10 mg.

Do not assume that this strength is appropriate for children.

Paediatric treatment should be based on:

  • Age
  • Weight
  • Diagnosis
  • Individual medicine strengths
  • Approved prescribing information

A healthcare professional should determine the appropriate paediatric formulation and dose.

15. Driving & Operating Machinery

Fexofenadine is generally considered less sedating than older antihistamines, but some individuals may experience dizziness or drowsiness.

Patients should determine their individual response before driving or operating machinery.

Avoid activities requiring full alertness if dizziness or drowsiness occurs.

16. When to Seek Medical Attention

Seek immediate medical attention for:

  • Difficulty breathing
  • Swelling of the face, lips, tongue or throat
  • Severe allergic reaction
  • Fainting or severe dizziness
  • Significant behavioural changes
  • Hallucinations
  • Severe depression
  • Suicidal thoughts or behaviour

Neuropsychiatric symptoms are an important warning associated with montelukast therapy.

17. Storage

Store ALERWITH-M:

  • At the temperature specified on the product label.
  • In a cool, dry place.
  • Away from excessive heat and moisture.
  • Protected from direct sunlight.
  • Out of reach of children.

Keep tablets in their original packaging until use.

19. Why ALERWITH-M?

ALERWITH-M combines two pharmacological approaches:

Fexofenadine

Provides H1-antihistamine activity to control histamine-mediated allergy symptoms.

Montelukast

Provides leukotriene-receptor blockade to reduce leukotriene-mediated inflammatory effects.
